BARKSDALE AIR FORCE BASE, La. -- It is time once again to send the kids back to school. If your child needs a school physical the 2d Medical Group has added two additional days in August to help get these completed.
Both days are on Saturday, Aug. 11 and 25. Appointments are available from 0900-1130 on both days. Sports Physicals are by appointment only. To schedule call the appointment line at 318-456-6555.
Some important information to note:
-Only children 5-18 years old enrolled to the
2d Medical Group can be scheduled in these appointments. If your child has an off-base provider you will have to schedule with that provider.
- Please bring all forms with you that are required by the school or after school activity. Arrive at least 15 mins prior to your appointment so you have time to fill out your child’s information on the forms.
-If your child wears glasses you will need to bring these as well so that the visual portion of the exam can be completed.
